Repeat Prescription Request

Patients who require repeat prescriptions may be contacted by a doctor for a review if necessary.

You can request your prescriptions by completing the online form below, or alternatively, print this form (PDF) and send it to us by post or drop it in at reception. ​

• Please allow up to 3 working days for the prescription to be completed and sent to your pharmacy
• There is a fee for Private (non-GMS) prescriptions
• Some medications require a 6 or 12 monthly review or blood tests. In such cases, you will be required to make an appointment. In conjunction with best practice guidelines, we actively discourage prescribing benzodiazepines or sleeping tablets on an ongoing basis.
